The Covanta forms and certifications are in PDF format. If you are experiencing problems with these documents, we recommend that you download the latest version of Acrobat Reader.

General Instructions for completing the Materials Characterization Form (MCF):

MCF must be submitted with every request for disposal.

In addition, identify the appropriate waste category (Oily Waste, or needed certificate) and fill out the respective form.

If the waste will be disposed of in the state of California (Stanislaus facility), then California Destination Addendum Form must also be submitted.
